CONSENT:
Consent was obtained from the patient. Indications, risks, and benefits were explained at length. All questions were answered.

PROCEDURE SUMMARY:
The patient received [Ibuprofen 600 mg] while waiting to be seen.  

A time out was performed.  The patient was placed in dorsal lithotomy position.  The speculum was placed and the IUD string visualized.  Ringed forceps were used to grasp the IUD string.  The IUD was removed successfully.  Bleeding was minimal.  A pad was provided to the patient.

Complications:  The patient tolerated the procedure well without complications.

Follow-up: PRN.  Return precautions given.
